Each clinic is made of multi-disciplinary medical partners, who are all dedicated to ensuring access to healthcare services to school-aged uninsured, underinsured, and families new to Florida - within our nation's 8th largest school district with over 220,000 students.

Back-2-School Health Clinics

Our Back 2 School Health Clinics provide children with NO COST health services, including but not limited to, routine & recommended immunizations, school physicals, eye examinations, dental & hearing screenings, in addition to a free backpack w/ basic hygiene supplies; meeting all the needs to help students get off to a great school year.

Pre-registration is required for all routine immunizations, school physicals, or shot records transfers to Florida SHOTS Form DH-680.

Families can register at www.back2schoolhealthclinic.org

We do not offer Headstart or Sports physicals.
